Advice for appointment with neurologist

I am wanting to make an appointment with a neurologist.  However, I am a bit nervous to call an schedule an appointment in the sense of i don;t want to say:  Hey can I see Dr. xxxx I am anxious and worried that I may have a neurological disease or it may be my anxiety...
Any advice?

When I made my appointment, I told the lady my symptoms and just said I was tired of trying to figure it out on my own. They were very nice.

Tell them your symptoms, write them down if you have to, and trust in the fact that they are much smarter than you could possibly imagine a human being actually being.

When I make an appointment for a doctor I just say "Hi, I'd like to make an appointment to see Dr. Whatever" and give them my info.
