New Dx GAD with Classic OCD and ADHD

Try to keep it short....  But this may get long.  I'm 36 yo male btw

Basically had anxiety my whole life.  Also took something as a child my parents called my vitamin. Had to take twice a day.  Did special classes after school with a couple other kids.  Basically I believe it was therapy.  My parents never told me what any of it was.  Eventually the vitamins stopped.

All through high school, college and career been struggling.  Motivation was to be number one at everything and that helped me in guess justify the way I am.  Took ephedrine and lots of coffee in college. It calmed my mind and let me focus. 

Never could do well with people in general and lose interest quickly in pretty much everything.  Always been an insomniac.  I figured I just didn't need a lot of sleep. Mom was same way so it wasn't odd to me

Finally the last 8 years or so it's really amped up and I finally went to see a dr about it a few months ago.  Got put on 40mg citalopram and eventually 7.5mg buspar.  Then also referred to a specialist .

Citalopram been helping even me out but still very fidgety and impulsive.  Still freaking at night about widows locks rain etc

New dr took make off buspar which didn't seem to do much and added 1mg tenex and 25 mg elavil to my meds.  Been on that for 1 week.  Tenex makes me NOT sleep when taken at night so I started today taking in morning .  Seemed like it helped my be calm till about 2pm. Then the nervousness and impulses came back slowly.

Not real sure what the elavil at low dose is supposed to do other than sleep

I'm thinking that the tenex is helping already but may need a second daily dose or a higher dose .

Also everything I've read is tenex doesn't help with attention/focus and today I will have to say that is about all I struggled with after switching the tenex to a morning dose

Does this all make sense?  Tenex is kind of helping during morning and at night if take after 6pm I have problems sleeping?  From what I read it is suppossed to help with sleep but seems opposite for me.

Also would it be out of line to ask doc to try stattera eventually?  Seems like tenex and strattera work on both side of ADHD. And if so can I stay on citalopram ?

I'm all for not taking a stimulant right now.  Besides I still get a slight calm from caffeine pills and occasional ephedrine.

Besides I still get a slight calm from caffeine pills and occasional ephedrine.

If you find caffeine pills and ephedrine calming then it may be that the only med you require is one of the stimulants used to treat ADHD such a Adderall and Ritalin, though it is definitely worth trying Strattera.

So since it's been a week on the new meds, I'm not familiar with them and their timing,  But I'm seeing some positive from them and I know with my SSRI and TCA that they take time to build up.  However the Tenex is helping a little bit. 

What are some opinions on when is it appropriate to call doc back?  I have a follow up schedule first week in June.  I can probably ride it out as I've already struggled most of my life with no meds to speak of.    Mainly I am curious if 2x daily dosage of tenex is ok or if I need a bump in mg.    Not sure I want to ask for strattera yet or any other med at this point, but the tenex does seem to be helping even though it feels like my focus is getting worse, but that' probably because it's the most glaring thing right now.

Thing is I don't want to seem like a drug seeker.  I just am frustrated cause I can see the light at the end of the tunnel finally and I'm excited and impatient.

I think you need to give the new med regime at least a month to settle down so unless you run into problems I'd leave it until you see your doctor again.

1mg/day is the recommended initial dose. If there is an insufficient response after 4 weeks then it can be increased to 2mg/day. Side-effects may begin to become at issue at 3-4mg/day.

Thanks for the advice.  I am just so happy to have some relief ya know.  I'll wait it out unless it becomes debilitating, but again I've gone a long time with this so a couple more weeks I think are doable.
