So glad to hear you're doing better!  I was going to suggest Cognitive Behavioral Therapy, but it looks like you're on it!  That's what helped me, after introducing a lot of behavioral changes like stopping drinking alcohol and caffeine, mindfulness, getting good exercise and good sleep.  Anxiety can be so powerful, it creates physical symptoms, and it sounds like you really had your share of those.  I can so relate to everything in your post, and my heart goes out to you for your bravery in fighting it!  Keep up the good fight.
