Hormone shots? I haven't come across this, and from my understanding of the topic I'd be surprised if this was a solution.
Generally I think there are two causes in OCD. In traditional OCD neurological investigations have revealed abnormalities in an area of the brain known as the basal ganglia. The basil ganglia is the part of the brain that is involved in habit. When we complete a routine acton, the basal ganglia triggers to let us know that the action has been completed. A fairly convincing theory goes that a malfunction in the basal ganglia means that certain actions (such as flicking a light switch, cleaning etc.) has not been satisfactorily accomplished. This can then have further psychological implications and can lead to high levels of anxiety and, in some instances, delusional thinking.
Pure-O, however, does not really have these traits. Here there is no routine task to be accomplished. I think, then, that while it is associated, this type of OCD perhaps has different origins to the traditional forms. Here it appears that anxiety is the precursor rather than a neurological abnormality in the basal ganglia, and that sympathetic dominance and perhaps (this might be where your hormones come in) elevated stress leads to a derangement of brain function. A threat is identified and thus we attempt to solve the threat. However, the nature of pure-O is that there can be no certainty and thus solving, and so what we are actually doing is digging a deeper hole for our perceived threat to entrench itself in. It is very, very similar to severe hypochondria though the connections are rarely drawn.
It is interesting also that rape and violence are essentially embedded within our DNA, and so the experiences of pure-O are essentially inescapable. This is a serious problem as people are desperately attempting to reject their own true nature. That is not to say that all people who imagine children naked are pedophiles, for instance, rather it is to say that humanity in general is somewhat depraved and so to deny all socially inappropriate thoughts is a sure method to the madhouse. Here I think you are correct, that pure-O is socially produced as it is immense social pressure exerting on our innate human instincts and desires which causes the immense anxiety. If being gay, a pedophile, a murderer etc., weren't socially outlawed then such arising thoughts would not produce anxiety.
This is not to justify in any way behaviour that is harmful. However people are generally (relatively) in control of their actions, whereas their thoughts are more of a free-for-all. It is not what you think that counts, it's what you do. Again, Tina, another perfect instance where mindfulness is a direct antidote to our struggles here at anxietyzone: by accepting what inherently is, we free ourselves from endless struggles. The author of that article, I think, communicates this quite succinctly.
